DT Swiss Centerlock 6 Bolt ISO Adapter
An adapter that lets a six-bolt ISO rotor mount to a Center Lock hub. The two rotor standards are not interchangeable on their own: Center Lock uses a splined interface and a lockring, six-bolt uses six small bolts on a flange. This bridges them in the one direction that is practical.
What it does
It is worth having because rotors outlive hubs. A new wheelset on Center Lock hubs does not have to mean new rotors, and on a bike with matched rotors front and rear it saves replacing a perfectly good pair to satisfy an interface change.
There is no adapter the other way. A Center Lock rotor cannot be made to fit a six-bolt hub, which is worth knowing before you buy rotors for a wheel you have not built yet.
DT Swiss is a Swiss company whose reputation was built on spokes and then extended to hubs, and its components share one habit - they are designed to be opened. That is why there is a catalog of service tools at all; a sealed part would not need one.

Carbon rims and carbon wheels bought new since 1 January 2020 carry an extended guarantee to the first owner against defects in material and workmanship, and DT Swiss runs a paid crash-replacement scheme on carbon wheels for up to ten years from purchase.

Can I go the other way, six-bolt hub to Center Lock rotor?
No. The adapter only works in this direction. If you own Center Lock rotors you want Center Lock hubs.
Does it affect rotor alignment?
It adds a defined thickness, which is accounted for in the design. Fit it properly and torque the lockring and the rotor sits where it should.
